Pre-Qualification Survey

Was your military service discharge classified as Honorable or Under Honorable Conditions (General)?

Were you released from active-duty within the last five years?

Was your final pay grade at or below E-5 or O-3 or W-2?

Were you promoted to E6/O4/W3 within six months of discharge?

Were you wounded, as defined in 10 U.S.C. § 1129 and recorded by the Department of Defense in a DD Form 214 or other official documents OR do you have a service-connected disability rating of 30% or greater as documented by the Department of Veterans Affairs?

Unfortunately, based on the information you have provided, we regret to inform you that you do not meet all the necessary requirements. The Stars of Valor Fellowships Program is intended for veterans with Honorable or Under Honorable Conditions (General) discharges within the last five years, who are at or below specified pay grades, and who are wounded or disabled as defined by the Department of Defense or the Department of Veterans Affairs.

Are you a widow or widower (remarried or not) of a deceased servicemember or veteran who died of service-connected injuries; or a parent (mother, father, stepmother, stepfather, mother through adoption, father through adoption, and foster parents who stood in loco parentis) of a deceased servicemember or veteran who died of service-connected injuries; or a child, stepchild, or an adopted child of a deceased servicemember or veteran who died of service-connected injuries; or a sibling (brother, sister, half-brother, or half-sister) of a deceased servicemember or veteran who died of service-connected injuries?

Do you have documentation of kinship, such as a marriage license or birth certificate?

Do you have proof of the servicemember’s death in the line of duty from the Department of Defense or proof of death due to service-connected injuries from the Department of Veterans Affairs?

The Stars of Valor Fellowships Program is intended for Gold Star family members who are related to a deceased servicemember or veteran and can provide documentation of kinship and applicable documentation pertaining to their death.
